ICAM Lead Investigator

Students who are deemed competent will receive a Statement of Attainment recognised under the Australian Qualifications Framework. This certification is nationally recognised. The Statement is issued by Pinnacle Safety and Training (RTO 40496).

The Statement of Attainment will include specific wording indicating that the participant has successfully completed the ICAM Lead Investigator course.

Assessment for this course includes both theoretical and practical components to ensure participants are competent in all aspects of the requirements for an ICAM Lead Investigator.

To be deemed competent, participants must demonstrate effective application of the ICAM methodology in a simulated assessment and produce a comprehensive investigation report. 

The theory assessment verifies understanding of key concepts such as WHS legislation, roles and responsibilities, hazard identification, risk assessment processes. Participants will develop a comprehensive investigation report that reflects their findings during the practical assessment scenarios.

The practical assessment takes place as a group activity in the virtual training environment, where participants must demonstrate their ability to apply the ICAM methodology. This involves forming an investigation team, collecting evidence using techniques such as PEEPO, and conducting witness interviews. Participants will then analyse contributing factors, identify root causes, and develop appropriate corrective actions, showcasing their practical understanding of ICAM principles.

All assessment tasks are accessed and submitted via a learning management system during class time.

This course is ideal for existing workers who have had exposure to working in and around confined spaces. 

The virtual classroom delivery option is ideal for those unable to attend the face-to-face course at our training facilities. This allows remote participants to complete the ICAM Lead Investigator course from anywhere, while still receiving the same high-quality, trainer-led instruction.

For those who are new to the workforce or whose roles do not involve leading ICAM investigation teams or require the ICAM accreditation, we recommend completing our ICAM Facilitator course. Use the quick reference guide below to determine which course is right for you.

There are no formal entry requirements for this course; however, we recommend that participants have some experience with incident investigations to get the most from the training. This could include having completed Pinnacle’s ICAM Facilitator course, possessing real-world experience in workplace incident investigations, or having undergone prior ICAM training. 

Age requirements

Participants must be at least 18 years of age. Due to course restrictions, enrolments will not be accepted from individuals under 18 years of age, even with parental or guardian consent. Refer to our FAQ for more information.

Language, Literacy, Numeracy and Digital skills (LLND)

Participants are required to have at least a Year 12 level of numeracy, literacy and communication skills and be able to read and write in English. This course requires a strong level of computer literacy to navigate the online learning system and complete required activities.

Recognition of Prior Learning 

Pinnacle Safety and Training does not offer Recognition of Prior Learning (RPL) for this course. Due to the duration and cost of this short course, RPL assessment would have limited value to participants. 

Credit Transfer

Pinnacle Safety and Training offers Credit Transfers for this training product. A Credit Transfer is granted where the previously attained unit of competency has been achieved. After enrolment, participants are eligible to submit a Credit Transfer application.

It is important to note that while the unit of competency is eligible for Credit Transfer, the participant will receive a Statement of Attainment without the specific ICAM Lead Investigator accreditation listed. This is due to the course being delivered in a contextualised format with specific methodologies, and course completion through attendance is required to achieve the intended learning outcomes.

All training and assessment requirements are completed within the course duration. Participants are not required to undertake any additional study or assessment outside of the scheduled course time.

​A Virtual Classroom is an online learning environment that allows for live interaction (audio and video) between a trainer and their students as they are participating in learning and assessment activities.

What’s the difference between Face-To-Face and Virtual Classroom delivery?

Choosing the right delivery format comes down to a few key factors. While both our face-to-face and virtual classroom options are led by experienced trainers and assessors—it’s important to consider your individual learning preferences when deciding which mode is right for you. Ask yourself:

• Can I easily get to a Pinnacle Safety and Training location?
• Do I prefer learning on a computer or using paper-based materials?
• Am I more comfortable in a physical classroom or an online environment?

When considering our Virtual Classroom option, it's important to make sure you have the right setup to get the most out of the experience. Ask yourself the following:

• Do you have a modern laptop or desktop computer?
• Are you able to attend from a quiet, distraction-free space?
• Do you have a headset or headphones with a microphone that work with your computer?
• Does your computer have a functional webcam?
• Do you have a reliable, high-speed internet connection that can support video streaming?

If you answered “no” to any of these questions, we recommend attending one of our face-to-face sessions instead to ensure you have the best learning experience. Participants who elect to undertake their training via Virtual Classroom must agree to all terms and conditions pertaining to this mode of delivery.

ICAM stands for Incident Cause Analysis Method. 

ICAM is an investigation methodology for use in a whole range of industries, including aviation, transport, mining, rail, security and health and draws on the work of Professor James Reason.

This could include having completed Pinnacle’s ICAM Facilitator course, possessing real-world experience in workplace incident investigations, or having undergone prior ICAM training.

After completing the ICAM Lead Investigator course, you’ll be equipped with the advanced skills and knowledge to lead complex incident investigations using the ICAM methodology. You’ll be able to:

• Lead or manage end-to-end investigations across safety, environmental, quality, and operational incidents
• Coordinate investigation teams and ensure a structured, evidence-based approach
• Identify root causes and systemic issues that contribute to incidents
• Develop and present clear reports with meaningful corrective actions
• Contribute to safety leadership and continuous improvement within your organisation
• Enhance your professional profile for roles in WHS, compliance, risk management, or operations leadership.
