ICAM stands for Incident Cause Analysis Method. ICAM is an investigation methodology for use in a whole range of industries including aviation, transport, mining, rail, security and health and draws on the work of Professor James Reason.

This ICAM Lead Investigator course has been developed to extend the knowledge and skills gained in Pinnacle’s ICAM Facilitator course, and is also suitable for those with existing real-world experience in incident investigations in the workplace, or prior ICAM training.

This course covers key incident investigation concepts and techniques designed to equip learners with the skills and knowledge to efficiently and effectively investigate workplace incidents, identify root causes, and recommend effective corrective actions to prevent future incidents.

  • ICAM methodology
  • Human and organisational factors leading to incidents
  • Safety culture and organisational drift
  • ICAM analysis on contributing factors including organisational factors, task/environmental conditions, individual/team conditions and absent/failed defences
  • Creating an investigation action plan
  • Duties under health and safety legislation
  • Incident response processes and procedures
  • Planning an investigation and forming an investigation team
  • Evidence collection methods and techniques, including the PEEPO Data Collection Matrix (People, Equipment, Environment, Procedures and Organisation)
  • Witness interview techniques and practices
  • Establishment of a timeline
  • Analysis of evidence to identify the root cause and contributing factors

​A Virtual Classroom is an online learning environment that allows for live interaction (audio and video) between a trainer and their participants as they are participating in learning and assessment activities.

A number of important factors must be considered in determining which delivery format may be right for you. These include (but aren’t limited to):

  • Computer hardware, peripherals and environment
    • Do you have a modern laptop or desktop computer?
    • Can you ensure you are in a quiet room for the duration of your sessions?
    • Do you have a headset or headphones with a microphone (such as those supplied with mobile phones) compatible with your computer?
    • Do you have a webcam?
    • Do you have a reliable and fast internet connection suitable for streaming video?

If the answer to any of the above questions is “no”, we recommend attending one of our face-to-face sessions. Participants who elect to undertake their training via Virtual Classroom must agree with all terms and conditions pertaining to this mode of delivery.

While in class (either in-person or via Virtual Classroom), you will research, prepare and submit your assessments through the Learning Management System (LMS), Moodle. You will spend time researching online, developing procedures and completing quizzes and long answer questions.

This program is only recommended to those with a strong level of computer literacy.

To be successful in this course you need to:

  • a) Have completed Pinnacle’s ICAM Facilitator course, or
  • b) Have existing real-world experience in incident investigations in the workplace, or
  • c) Have undertaken prior ICAM training.

     
  • Be at least 18 years of age
  • Have at least a Year 12 level of numeracy, literacy and communication skills
  • Have the ability to read and write English. If English is a second language, you will need to complete a language, literacy and numeracy (LLN) questionnaire. Contact us to discuss
  • Use a device that meets the minimum System Requirements of Pinnacle’s e-Learning system (we officially support and recommend modern desktop or laptop computers running Google Chrome web browser).
  • Have a strong level of computer literacy skills. Your assignments will be completed on a computer, and if you do not have a suitable level of computer experience, you will not be able to complete the course in the timeframe allotted
  • Be confident navigating with web browsers
  • Have the skills to compile electronic documents using software applications (word processing)
  • Be able to conduct research and present information
  • Agree with all terms and conditions of your enrolment

For our public (face-to-face) courses at our purpose built training facility trainees must provide:
